What happened:
Wireline crew informed in the lesson learned meeting that it was some confusion on when to fire jar and who should be notified/approve.
What was planned:
In DOP; to inform/confirm the DSV prior to fire jar for wireline operations
What was learned:
That it should be stated clearly in the DOP when to fire jar and if anyone should approve. In case of stuck string the string can get even more stuck if time goes by if unable to reach DSV to approve
What actions are required:
Clearly state in the DOP the jarring procedure for wireline operations, when to fire and who to approve.
