What happened:
During operation several MoC’s were made. A link to the risk register was enclosed in the MoC report. The link was to the risk register for the well. It was then difficult to identify the risk related to the MoC and who participated in that risk assessment.
What was planned:
Preparation of MoC with Risk assessments
What was learned:
A seperate risk register shall be attached to the MoC report.
What actions are required:
Inform all Lead engineers how to prepare MoC. Prepare a course for relevant personnel.
