There was a dropped object (DO) incident onboard BGL, and DO is not included in the reporting & notification matrix.
What was planned:
The operational interface document have a reporting & notification matrix to be used as decision tool for reporting when an incident occurs.

The importance of having a reporting & notification matrix with the needed information. How to act on any incident.
What actions are required:
– Reporting & notification matrix to be updated and implemented in coming operational interface document – Include the formula for calculating fall energy
